This form comes with a job application pack. Equal Opportunities is all very well and dandy in order to give everyone fair chances. However such policies discriminate against someone who does not fall into a category group, which is not fair. I myself am an average person and feel that I have not been getting a job because I am not considered a minority. I feel the best person should get the job and if they happen to be part of a minority group then that is a bonus. Next time I fill in one of these forms, I am actually going to tick the 'disability' box. I am sure that doors will open for me...You mark my words...
